Frequently Asked Questions:
How do I schedule an appointment?
You can schedule an appointment with your physical therapist by using the numbers below:
- Blaine Clinic Physical Therapy (763) 528-2992
- Brooklyn Park Clinic Physical Therapy (763) 569-6202
- Columbia Heights Clinic Physical Therapy (763) 782-8106
- Fridley Clinic Physical Therapy (763) 572-5706
What should I bring to my appointment?
Be sure to bring your physician referral and list of medications. We do supply paper gowns and shorts. However, if you are being seen for low back, hip, knee, ankle, or foot, we encourage you to bring your own shorts. If your physician has ordered iontophoresis, be sure to fill the dexamethasone prescription and bring this with you to your first visit.
How long will my appointment take?
Your evaluation will be approximately 45 min – one hour. After that most appointments are about 30 minutes. Your therapist will discuss this with you.
Will my insurance cover therapy?
It is every patient’s responsibility to check their insurance to determine their covered benefits and co-pay for therapy. You may want to call the customer service phone number on the back of your card before your first appointment.
What if I need to cancel an appointment?
In order to achieve maximum results it is best to attend each appointment. However, if you need to cancel an appointment please call 24 hours in advance. If you are calling outside of clinic hours, please leave a voicemail message with your name, time, and date of appointment.
- If you miss three appointments without notifying the clinic, subsequent visits will be cancelled and you will need to see your physicians before scheduling further Rehab appointments.
- If you miss multiple appointments we may ask you to schedule one visit at a time.
Will I see the same person each time?
Your therapist will discuss this with you at your first visit. Your likely will be working with a primary therapist and an assistant. Please keep us informed of your follow up visits with your referring physician. Keep in mind any physical therapy rechecks before a physician follow-up and your final visit should be scheduled with your primary therapist.
Can I bring my kids to the appointment?
We are unable to provide daycare during your appointment. If at all possible, it is best not to bring children to your appointments. If this is an issue for you, please discuss this with your therapist.
Can I be seen at other locations?
Physical Therapy is located at our Blaine, Brooklyn Park, Columbia Heights and Fridley Clinics; you are welcome to be seen at any location. However, we do ask that you choose one location for all visits.
