To leave a message for your clinician, please call our messaging staff at (763) 572-5700 and press 5.
To Cancel an Appointment:
All canceled appointments will need a 24 hours notice. To cancel an appointment call (763) 572-5700 and press 3 to reach the cancellation mailbox. After the beep, leave the patient's name - spelling out the last name, the date of birth, the clincian's name, the date and time of the appointment, and your telephone number.
To Request a Re-fill on Your Prescription:
If you need a re-fill on your prescription, please call your pharmacy directly.
